PROGRAM SYNOPSIS
In recent years it has become well accepted that direct composite resin restorations are predictable in small to medium sized cavities; similarly, the durability of full coverage crowns is well known. However, the grey area is when a cavity falls somewhere between the two: what do we do when a direct restoration would seem wildly optimistic, yet a crown overly destructive? This unique program addresses the theoretical aspects of reconstructing an entire arch with direct composite resin from planning, through placement and final finishing using a no or minimal preparation approach.

COURSE OUTLINE
The two day hands-on component allows the delegate to restore an entire half arch — incisors, canine, both premolars and a molar — using various direct and semi-direct approaches under close supervision.

DAY 1 - Theory

  • Direct composite resin veneers: how to reconstruct aesthetics and anterior guidance for the complex case.
  • The Dahl Concept: approaching full mouth rehabilitation in a conservative manner.
  • An overview of Bio_Emulation.
  • The “Peripheral Rim Theory” and the importance of Rainey’s Ridge and Web in preparation design.
  • Minimally invasive cavity preparations. Why preservation of tooth structure is critical to long-term success. Marginal ridge preservation and tunnel preps.
  • How to avoid collateral iatrogenic damage to adjacent teeth.
  • Particle abrasion and sonic preparation/cementation.
  • The use of RMGIC and dual cure composite as “Mega-Fillers” to reduce cuspal flexure and potential fracture in wide cavities.
  • The GIC open, closed and super-closed sandwich techniques: when, where and how.
  • The chairside semi-direct inlay/onlay as a strategy for the restoration of structurally compromised teeth.
  • Immediate Dentin Sealing and Gingival Margin Elevation: New concepts for the 21st century.
  • Preparation designs and instrumentation for partial coverage adhesive restorations.
  • And much much more……………

DAY 2 - Hands On - Establishing Anterior Guidance
The first day of the hands-on program involves establishing anterior guidance on typodont teeth. Each delegate will restore the palatal volume and incisal edge of the central and lateral incisor with direct composite resin veneers. Two veneering techniques will be demonstrated and practiced: “The Natural Layering Concept” and a more simple approach utilizing external tints. The last exercise of the day is a simple rapid technique to restore worn canine cusp tips. The emphasis of the day is functional aesthetics.

DAY 3 - Hands On - Creating Posterior Support
The day begins with each delegate rebuilding first and second premolar palatal cusps with direct composite in order to increase the Occlusal Vertical Dimension. A technique to accurately transfer the occlusal scheme from the diagnostic wax up to the mouth will be demonstrated. Delegates will then carry out this procedure on their own model. Dr Smithson will then demonstrate an MOB “Semi-Direct” composite inlay/onlay replacing a cusp, from bonding/core build-up/ preparation through impression/model fabrication and finally fabrication and cementation. The delegates will then get the opportunity to fabricate their own onlay with close support from Jason.

About Dr. Jason Smithson
Jason Smithson BDS DipRestDent RCS (Eng) qualified at the Royal London Hospital in 1995 and the Royal College of Surgeons in 2008 having a special interest in direct composite resin artistry and minimally invasive all-ceramic restorations. He has presented to dentists internationally in the USA, Canada, Australia, New Zealand and throughout Europe, as well as being involved in postgraduate dental teaching in both the Cornwall and Plymouth Foundation Dental Practitioners Schemes. Jason is also a clinical lecturer at The Peninsula Dental School, UK and Honorary Clinical Tutor in Restorative Dentistry at Warwick University and is a member of the Bio_Emulation Think Tank . He practises at Pure Dental Health in Truro, Cornwall.
