Deep margin elevation is a treatment approach to relocate the cervical margin of a tooth with subgingival defects to a supragingival location. This technique utilises a direct restorative material to facilitate better clinical outcomes in restoring complex class II cavities whether done in conjunction with indirect restoration or direct composite restorative option. Proper case selection, effective isolation and reliable adhesion are key factors to make this viable treatment approach to deliver favourable outcomes. The aim is to improve the integrity of the tooth being restored to have effective marginal seal and adhesion providing a sound substrate necessary beneath a ceramic or composite restoration to retain the tooth in the long term. DME is a concept that every clinician should know and understand. In this lecture we will discuss 5 key tips and tricks to carry out this technique.
