The vertical dimension of alveolar bone available for implant placement in the posterior maxilla is determined by a) the degree of pneumatisation of the maxillary sinus, b) amount of crestal alveolar bone loss, or c) a combination of these two factors. If the degree of maxillary sinus pneumatisation is the primary reason precluding conventional implant placement, then regeneration of the maxillary sinus floor is indicated. If the patient has around 4mm of vertical bone present at the desired implant site, then a closed (transcrestal) sinus lift may be an option. However, when less than 4mm of vertical bone height is available, a lateral window maxillary sinus floor augmentation procedure is the treatment of choice.
