Vertical bone augmentations in dental implantology still represent challenging procedures. Onlay grafts using autologous bone or combinations with bone substitutes are common with a clear shift away from extraoral grafts to local intraoral grafts. CAD/CAM fabricated titanium meshes help to reduce surgery time and improve accuracy of the graft position in relation to the planned supraconstruction. With the introduction of Piezosurgery and the experiences from distraction osteogenesis interpositional grafts using Bio-Oss blocks have become a standard in their respective indication. Bone augmentation procedures also need to address the soft-tissue situation and ensure soft-tissue stability around the planned implants. In the clinical situation, with the promising survival data of short implants, it is important to include the patient into the decision for the best procedure.
