Even though no definitive numbers can be given, it should be clear, by now, that periodontally compromised patients must be informed that they are at higher risk of peri-implant disease, before treatment is initiated.  When peri-implantitis has already caused extensive bone resorption, the dentist is faced with the dilemma of whether regenerative therapy is appropriate in order to maintain the implant, or whether removal and replacement with a new implant would be preferred.
Even though no surgical technique is currently available to predictably regain osseo-integration, a novel protocol for hard tissue reconstruction will be described. The positive results in regenerative surgical treatment of peri-implantitis are based on: 1. defect selection, 2. patient preparation, 3. surgical flap management, 4. post-operative care. A novel decision tree will be shared. Long-term data will also be presented to support
the efficacy of this treatment modality.
