Whilst there is a general apprehension of visiting dentists, in part related to the issue of pain, for other patients there are much greater challenges. Individuals with posttraumatic stress disorder, particularly if they have been subject to inter-personal violence, the experience of the physical proximity required by dental therapy represents a major challenge. Paradoxically, these patients are likely to be over-represented in dental practices because posttraumatic stress disorder is associated with a variety of dental problems such as temporomandibular joint dysfunction, bruxism and headaches. These are a direct consequence of the psychophysiological abnormalities associated with posttraumatic stress disorder. This symposium will outline the clinical features of posttraumatic stress disorder and how they might manifest in a dental setting. The broad principles of aetiology will also be described. The basic principles of treatment will be described and in particular, management strategies that may assist in anticipating and managing the anxiety of this particular group of patients when they seek dental treatment.

Practical information will be discussed against a background of the neurobiology of posttraumatic stress disorder and its impact on physical comorbidities. This body of knowledge provides invaluable insights into the pathogenic affects of stress in both physical and psychological health.
